For Registered Nurses ·
What you'll accomplish
By the end of this guide, you'll understand what ambient AI documentation tools (AI scribes) are, which ones are being deployed for nurses specifically, whether your hospital is likely to implement one, and how to use a free workaround approach while waiting for institutional deployment.
What you'll need
Ask your nurse manager or informatics department: "Is the hospital piloting any AI documentation tools for nursing? I heard about Abridge and Nuance DAX for nursing notes."
If yes: Ask to be included in the pilot program. Early adopters shape how these tools are configured.
If no: You can plant the seed. Reference Vanderbilt, UNC Health, and CommonSpirit Health — all have deployed nursing ambient documentation with documented time savings. The ROI for hospitals is significant (every minute of documentation time saved × 3 million RNs = massive efficiency gain).
What ambient documentation does: Listens to your assessment conversation with the patient. Generates a structured nursing note (SOAP/assessment format) in draft form. You review, edit, and sign.
What it doesn't do: Replace clinical judgment. Make decisions. Access other patient records. Work outside of a direct nurse-patient conversation.
Accuracy reality: Current tools are 70–90% accurate for nursing notes — meaning you still review and edit every note. The time savings come from not typing from scratch.
While waiting for institutional ambient tools, here's a free approximation that many nurses find genuinely useful for documentation drafts:
Method: After completing a patient assessment (not during — you don't want to be distracted), use your phone's voice dictation to record a quick verbal summary of your assessment findings. Then paste the transcription into Claude or ChatGPT to structure it into a proper nursing note format.
Important: This voice-to-text approach is for your own personal device and must use only de-identified information — no patient names or MRNs.
When ambient documentation rolls out at your facility, here's what to expect:
Ambient documentation tools improve based on correction data. Every time you edit the AI-generated note, you're teaching it. Be specific in your corrections — change the actual words, don't just delete and retype from scratch.
Convert voice note to SOAP: "Convert this nursing assessment dictation into a SOAP note: [paste transcription — de-identified]"
Structure a verbal report: "Organize this into a shift handoff note: [paste dictation]"
Summarize shift events: "I described these events verbally during my shift. Organize them into a chronological nursing note: [paste transcription]"
